CONDITIONS AND SYMPTOMS ASSOCIATED WITH HORMONE IMBALANCE Dr. Tano has had intensive training in hormone imbalance and consequences by the Panamerican Allergy Society. He has since dedicated countless hours to understanding the pathophysiology of diseases or medical conditions that have their etiology in hormone imbalance such as obesity, diabetes, hypertension, hypercholesterolemia (hyperlipidemia in general), the disturbances in female menstrual cycle: irregular menses, heavy menstrual bleeding, dysmenorrhea (painful menstrual cramps); many other chronic conditions such as: ovarian cysts, fibrocystic breast disease, polycystic ovarian syndrome (PCOS), fibroid tumors and associated heavy bleeding and anemia, endometriosis, acne, premenstrual headaches (migraine), premenstrual asthma, premenstrual urticaria (hives), premenstrual syndrome or premenstrual dysphoric disorder (PMS, PDD), mood swings, depression, anxiety, panic attacks, hot flashes, fibromyalgia, unexpected hair loss, low libido in both men and women... ORIGIN OF HORMONES, THEIR INTERACTIONS AND CONSEQUENCES Our good hormones come from LDL cholesterol which generates Pregnenolone. Pregnenolone divides into two other hormones, Progesterone and DHEA (DeHydroEpiAndrosterone); these two in turn produce estradiol, Estrone and Testosterone. When we get older, we lose some of these hormones and others increase. When progesterone decreases, thyroid function decreases which leads to weight gain. Unfortunately, TSH measured by most physicians to test for the thyroid function, often, does not capture this imbalance. The decrease in progesterone also leads to an increase in INSULIN, which is the number one fat maker in the body. Initially, when insulin increases, the blood glucose decreases which prompts craving for carbohydrates (sugar containing foods). In order to replenish the decreased glucose, Adrenaline is also produced and most people become "hyper". When glucose is plenty, the insulin puts it into fat cells and people start gaining weight. When the fat cells are filled, they signal to the brain that they have enough of the glucose by producing hormones such as LEPTIN(BIOS LIFE SLIM-listed in the Physician Desk Reference-PDR- boost the LEPTIN so as to communicate better with the brain), Adiponectin, Visfatin and Apelin which are to tell the hypothalamus to put a break on the appetite and sugar cravings. After a while, the message is lost and most people continue to gain the weight. When the fat cells are filled and do not want the glucose, this glucose spills in the blood and high levels of glucose in the blood is what is known as diabetes (insulin resistance). Fat cells also produce Adipokines such as TNF-alpha, Interleukin-6 and many others that lead to inflammatory processes such as asthma, allergic rhinitis and aches and pains and that's why most obese individuals experience these symptoms. The fat cells also produce estrogens (Estradiol and Estrone) which in turn increase the insulin level and decrease the thyroid function (by increasing the thyroid binding globulin) and therefore lead to more weight gain. Too much estrogen is not good for the body. For women, the decrease in progesterone and increase in estrogens lead to several benign tumors such as fibroid tumors, fibrocystic breast disease, ovarian cysts, polycystic ovarian syndrome and malignant tumors such as Breast, vaginal, cervical, uterine, ovarian, and colon cancers. Benign but devastating tumors such as fibroid tumors are source of heavy and often prolonged bleeding(dysfunctional uterine bleeding) and anemia which lead to hysterectomies(surgical removal of the uterus). Both men and women lose their testosterone with age and this leads to low libido, decreased muscle mass,increased insulin and abdominal weight gain. DHEA DHEA is the mother of all hormones and affects all other hormones in the body. It is also known as the longevity hormone. Studies have shown that people who have more of it tend to live longer on average than people who have less of it. A decrease in DHEA which occurs with age and stress has dire consequences on the overall well-being of humans. A decrease in DHEA leads to a decrease in testosterone and increase in insulin. An increase in insulin tends to cause not only diabetes, but also salt retention and hypertension. REASON FOR HIGH CHOLESTEROL When the hormones are low, the body wants to produce them and naturally raises the ingredient that is needed to make those hormones and that is why cholesterol increases. An increase in cholesterol should therefore be a signal to test for the hormones. CHOLESTEROL PRODUCTION AND CONSEQUENCES Cholesterol is made in the liver and out of fat and glucose, and insulin serves as the fuel to the rate limiting enzyme HMG-CoA reductase and that is why many diabetics have cholesterol problems and why obese men and women who invariably have hormone imbalance, also have fatty liver. In the making of cholesterol, occasionally cholesterol stones are formed and lodge in the gallbladder and gallbladder neck and prevent the flow of bile. This leads to cholecystitis (inflammation of the gallbladder) which is one of the main reasons for a cholecystectomy (surgical removal of the gallbladder). In medical school, we learned that cholecystitis is often found in "Females, Fat, Fertile and Forty". Cholecystitis in most women is therefore a sign of hormone imbalance. OUR INTERVENTION When we see patients in our clinic who have adult onset allergies, obesity and/or diabetes, hypertension, hypercholesterolemia, cholecystectomy, and total abdominal hysterectomy, and/or many of the symptoms mentioned above, we think of hormone imbalance and measure their hormones.
